What does "miscommunication" really mean??

We have heard it from the Packers coaches for years and years. Pass coverage repeatedly broke down or broke down at key times because of "miscommunication." Defenses have been gashed by running games because of "miscommunication." This past Sunday. O-line play broke down because of "miscommunication."

On the football field, what really happens when there is "miscommunication"? It seems that it can be any of several basic events:
- a call misunderstood or unheard by one or more players.
- seeing an opponent look and making a wrong call in response
- players at different levels making inconsistent calls
- payers failing to respond correctly or failing to convey a call

Apart from errors that happen in delivery and reception between a speaker and a listener, are not "miscommunications" mostly lack or failure in preparation?

Stenavich said specifically, in the Vikings game case, it was a failure between the line and the RB. Which is weird because Brooks has been rock solid for most of two years.

So what I would take from that is that the two newest guards and maybe Morgan had trouble with calls. However, in all the clips I watched the worst problems were the center and guard double teaming a player and someone shooting through the guard gap with NO ONE THERE. Happened at least three times.

On some plays, it did look like the Brooks didn't hear the protection call and missed picking up the man side rusher. BUT, there were also times were it looked the the guards forgot which side of the line they were on. C/T slide and G on that side thinks he is man and lets a free rusher thru ? Of all the teams to play week one, MN was the worse with all the blitz overloads they run. Not good that the next opponent blitzed nearly 45% of the time against TN.
